Hope Therapy Center LLC ("Hope Therapy Center," "we," "us," or "our") operates the website at hopetherapycenter.net and provides physical therapy, occupational therapy and speech-language pathology services in Lake Charles and Lafayette, Louisiana. This policy explains what information this website collects, how we use it, who we share it with, and the choices you have. It applies to this website only. Information you give us in the clinic as a patient is handled under our clinical privacy practices, which are separate from this website policy and are provided to you at your first visit.

Information You Give Us

The appointment request form on this site asks for your name, your phone number, your email address, and a short message describing what is going on and what your goals are. The form also includes a checkbox for text message consent. Those five items are the only fields on the form, and every one of them is submitted to us only when you press send. Please do not use the form to send us detailed medical history, diagnoses, test results, or anything else you would consider sensitive. A brief description is all we need, and the rest is better handled by phone at (337) 478-5880 or in person.

Information Collected Automatically

Like most websites, ours records limited technical information when you visit, including your IP address, your browser and device type, the pages you view, and the site or search that referred you. This is collected through standard cookies and analytics tools and is used in aggregate to understand which pages people find useful. You can block or delete cookies through your browser settings, though some parts of the site may not work as intended afterward.

How We Use Your Information

We use the information you submit to contact you about your request, schedule and confirm your appointment, send appointment reminders, answer your questions, and follow up on your care. Text Messages and Consent

Text message consent is collected separately and explicitly. It is a checkbox you tick yourself, it is never pre-selected, and leaving it unticked does not stop us from booking your appointment. The consent language on the form reads: "By providing my phone number, I agree to receive text messages from Hope Therapy Center. Message and data rates may apply. Reply STOP to opt out."

You can revoke that consent at any time by replying STOP to any text message we send you. You may also email frontoffice@hopetherapy.co or call (337) 478-5880 and ask to be removed. We do not sell or share your mobile number with third parties for their own marketing. Opting out of text messages will never affect your ability to book, attend, or continue care with us; we will simply reach you by phone or email instead.

Who We Share It With

We share information with service providers who help us run this website and reach you, and only to the extent they need it to do that job. These include our website host, the form submission service that delivers your request to our inbox, our email and text message providers, and analytics services. We may also disclose information where we are required to by law, in response to a valid legal request, or where necessary to protect the safety of a person.

Third-Party Reviews Widget

Pages on this site display our Google reviews through a third-party review widget that loads content from an outside provider. When that widget loads, your browser connects to the provider's servers, which means the provider can receive technical information such as your IP address and browser details, and it may set its own cookies. The reviews shown are published by Google users, not by us, and the widget's handling of your information is governed by the privacy policy of that provider and of Google, not by this policy. Nothing you enter on our appointment form is passed to the reviews widget.

Security

We use reasonable administrative and technical measures to protect the information submitted through this site, including transmission over an encrypted connection. No website or method of transmission over the internet is completely secure, so please do not send anything through this website that you would not be comfortable sending by email.

Children

We treat children in our clinics, and a parent or guardian is the one who books for them. This website is not directed at children, and we do not knowingly collect information submitted by a child through our forms. If you believe a child has submitted information to us, contact us and we will remove it.

Your Choices

You may ask us what information we hold about you, ask us to correct it, or ask us to delete what you submitted through this website. You may unsubscribe from marketing email using the link in any such message, and you may opt out of text messages as described above. Requests about your clinical records are handled separately under our clinical privacy practices, and our front office can walk you through that process.

Changes to This Policy

We may update this policy as our website, tools, or legal obligations change. When we do, we will revise the effective date shown at the top of this page. Continuing to use the site after an update means you accept the revised policy.
