The form below takes about a minute, and it goes to our own front office rather than a call center: fill it in and someone from the team phones you back during business hours to find a time at Lake Charles or Lafayette, whichever is closer to you. Your first appointment is a one-on-one evaluation with a licensed therapist who tells you what is actually going on, what it will take to change it, and what it will cost before you commit to anything. If therapy is not the right answer for your problem, we will say so and point you to who is.

The details below are our primary Lake Charles clinic. Lafayette, at 1512 Camellia Boulevard, runs Tuesday through Friday, 8:00am to 6:00pm, on the same phone number and the same email.

• No physician referral needed. Louisiana is a direct access state, so you can start physical therapy with us straight away. If your condition has not shown documented improvement within 30 days, we are required to refer you back to a physician, and we will help arrange that.
• Your benefits are checked before you come in. Our front office verifies your specific plan, deductible and visit limits ahead of the first appointment and tells you what you will owe up front.
• Sessions run 30 to 60 minutes, one on one with your therapist.
